ONE: Before Your Appointment

First, the paperwork. It’s a little lengthy, but for your benefit, we like to be thorough.

Please fill out and submit the following forms online before your appointment. This will help us prepare for your appointment and lessen your time here.

TWO: Bring the following to your first appointment

  • A list of medications, supplements, vitamins, herbs that you’re currently taking
  • Your physicians’ phone numbers (MD, chiropractor, acupuncturist – anyone you regularly see for check-ups and/or chronic issues)
  • Any x-rays taken from your previous dentist

Patient Forms

Infections and diseases of the mouth can affect the rest of the body and vice versa. We like to have a thorough understanding of your overall health before we start any treatment. We will also ask for your permission to speak with your other doctors to ensure more comprehensive treatment.

For our new patients we like to do the following:

  • Periodontal probing (gum/bone loss check)
  • Microscope plaque sample
  • Tongue examination
  • Oral cancer screening
  • Digital diagnostic X-rays (unless taken within 6 months)
  • Intra-oral digital pictures of your tooth magnified 100x
  • Discuss oral hygiene habits, techniques, and tips for improvement
  • Discuss treatment options (unless no treatment is necessary)

We use these diagnostics to create a snapshot of your oral health and to help keep your teeth healthy for the rest of your life.

If you had x-rays taken less than six months ago, new x-rays may not be necessary. It is advised to send all previous x-rays so we have a thorough record of your dental history. This way, if there is a questionable area, we can look back 1, 3, 5+ years back to see its progression. This can save you from future procedures.

After all information is gathered, a customized treatment plan is created for you. Together, we will work to improve or maintain your dental health. If any dental work is required, we suggest taking a biocompatibility test. This way, we ensure that you receive the proper filling, crown, implant, or any other material necessary for your body.
